Thank You for your interest in registration at our agency. Please select appropriate registration form. Form fill using computer and print it or print it and fill it by hand.  Do not forget to sign it.

Actor Registration
Being professional or amateur artist, select Actor Registration Form  (download rtf, doc, pdf).

Extras Registration
Being not an artist, select Extras Registration Form (download rtf, doc, pdf).

Child registration
Being younger 15 years, the form must be signed by responsible person (usually your parent).  Select Child Registration Form (download rtf, doc, pdf).
Being in age of 15-18 years, you can sign form by yourself.

After You fill the Form send it signed to address of our office:  ArtCorp s.r.o., Vlastina 889/23, 161 00 Praha 6.
Our worker will confirm your registration, alternatively will suggest you term for taking of photos.
